I am very frustrated in trying to obtain my other halfs $10 rebate form. He purchased his 3 pack of Rogaine @ Walgreens Pharmacy. He was told to go online and receive form. The only form I saw was a pdf form which I can not print and then an outdated site stating purchase has to be from 6/2013 thru 8/2013. Could you please email me form to address above so I can print it & mail in with his receipt. he is a long time user any $ saved helps. Thank you
